Dont know, my guess would be all skills. I look at it in this way, players would play at 100% of all their skills when in divine form.

edit: i'm sure someone here will have a better answer

my main concern is if my player has a hopeless skill level in stamina or pace does that mean they will be hella slow? or if they had better form they would play better than hopeless?

to be honest, when the skills are that low, form, I don't think, has that much of an impact. But when they start getting into the higher levels of skill is when you will start to notice worse play.

just think of divine form as the best a player can play with his current skill set

but your example is ridiculous because hopeless is god awful anyway. i told you shoot for ~average in any important skill for his position to start with minimum

haha i only have 1 guy who is that low but besides that im starting to get the hang of it no worries :D
